Saw palmetto is anti-DHT. DHT is the stronger form of testosterone and testosterone interferes with the effects of estrogen such as NBE. It also has a long history of use for NBE. DHT is also the type of testosterone that promotes acne.

progesterone => 17-OH-prog => androstenedione => androgens => estrogens
androgens = testosterone

Plus she started right away with a high amount of progesterone. It might be enough to reduce the progesterone temporarily until she gets more anti-testosterone herbs.
Progesterone counterbalances estrogen so it deals with estrogen dominance. Estrogen dominance is too much estrogen relative to progesterone.
