You have to do the inward movement when your breasts are up and the outward when your breasts are down, you know, in 360 degree. I hope with this answer that you can imagine how to do the chi massage.
Answering to your question, yes, one breast go to the counterclockwise direction and the other go in the opposite direction, normally when you are doing the inward movement the left breast touch the right (or viceversa XD), but in the outward they separate. If you don't know it you have to use some moisturizer when you does this massage, in this forum it's recomended cocoa butter with vitamin E, among other moisturizers, but you can use castor oil or olive oil, to tell you some examples.
