It's not painful for me either, and I use mine on level 3 for power and level 3 for the speed the entire session. The cream is to be put on the black part of the pads that go on the breasts, that is so it can work, just like with a tens unit you have to use a gel for it to work, same difference. I just use my regular body lotion and put a tiny bit on the black part before each use and clean them after. I usually use mine for 30 min, putting it on a different mode each time. So 2 sessions back to back.
