I've always had an insecurity because my nipples aren't super small and cute. They're not big but I'm afraid they will get a lot bigger w/ NBE. Part of the reason I want to do NBE is to make my nipples look better proportioned w/ my boobs.

Has anyone experienced any extreme changes in their nipples since they've started?

Yes absolutely they do, majority of the time anyway(both areolas and nipples)
Imo it's much better to have medium sized nips then underdeveloped pre pubescent nipples/areolas but that a personal opinion. Mine have grown alot of the years of NBE, it also depends on if you use BO or a herb that also boost prolactin(prolactin makes areolas and nipples bigger too)

I personally agree that it's better to have medium sized nipples than super small and cute. But having said that, there's no common size for nipples, they vary from person to person and all sizes are normal.
